The last in-person care was June 9th, 2025. Subsequent care was by Telehealth conference which ceased on August 31,2025 except for special transition of care circumstances. The office closed for all patient care December 31,2025. The business office can be contacted until December 31, 2026. Communication can be made with the business office by phone, fax, or email. The contact information is unchanged.
