Medical Records Request

Medical records are available until 12/31/2026 for former patients seen from 01/01/2021 to 12/31/2025.
A medical record request must be made in writing or emailed. Provide the healthcare provider's name, address, phone and fax numbers. Your current problem list, recent chart notes, medication and allergy logs and applicable lab studies will be sent at no charge. There is a charge for more extensive records request to a provider and personal copy records request.
