Medical record request must be made in writing or emailed. Provide the healthcare provider's name, address, phone and fax numbers. Your current problem list, medication and allergy logs and applicable lab studies will be sent at no charge. There is a charge for more extensive records request to a provider and personal copy records request. You will be notified in advance of the cost. Payment must be made in advance of release at the address below. Medical records deadline is 12/31/2025.
